We are happy to register you as a patient at Waterside Medical Practice if you live within our practice area. You can only be a registered patient with one GP surgery so if you register with us you will be de-registered with your current GP surgery and your previous medical notes will be sent to us.

Registration with the practice

If you wish to register please come into the surgery to complete a registation form. You will need to bring along two forms of ID with your current name and address. Such as a utility bill, driving licence, passport. The utility bill will need to be dated within the last three months to be valid.

When you register you will also be asked to fill out a medical questionnaire. This is because it can take a considerable time for us to receive your medical records.  When you come to the surgery you will be asked to sign a registration form.

You will need to book in with the HCA for a new patient check up prior to any medication being given.
